    By signing this application, I certify that the facts set forth or contained in this employment application are true and complete to the best of my knowledge.  I authorize investigation of all answers, statements and information contained herein.  Specifically, I authorize Henson Construction Co. to make all necessary and appropriate investigations or background checks to verify the accuracy of the information provided, and to obtain reference information of / on my work performance.  Furthermore, I authorize all persons, companies, corporations, schools, former employers, law enforcement agencies, state bureau of criminal investigators, doctors, physicians, departments of motor vehicle, automated workforce screeners or consumer investigators to supply or disclose any information concerning my background.  I release, discharge and hold harmless Henson Construction Co., and each of the above, from any and all liability, of whatever kind or nature, which could result from obtaining and having an employment decision based on such information.
