RELEASE, WAIVER OF LIABILITY

Do not sign this agreement unless you have read and understand it in its entirety.
This release will remain applicable and filed in your pet’s boarding chart for all future boarding stays.

Boarding Requirements: All pets must be current on Rabies Vaccine, DAPL Vaccine, Canine Bordetella Vaccine, K9 Influenza Vaccine, Annual Heartworm Test, negative Fecal Diagnostic Test (every 6 months) and proof of flea prevention. Be at least 4 months of age, be in general good health and free of ticks and fleas. All owners are held responsible for their pet during the check-in and check-out process. All pets must be leashed when entering and exiting VWCG facility.

Personal Property: (May have restrictions due to COVID) We encourage you to bring your pet’s favorite blankie, bed,and toys. Items that smell like home help your pet create their own “home away from home” environment while boarding. We strongly recommend that you label these items with both you and your pet’s names. We also suggest that no irreplaceable items be brought due to shifting of items for numerous reasons, we cannot guarantee their return. VWCG will not be liable for any lost, stolen, or damaged personal property belonging either to you or your pet.

Food and Treats: Your pet will be fed with the food and instructions you provide. Feel free to bring your own measuredand pre-packaged food with instructions for your pet’s entire visit. If your pet has a food allergy or is on a special diet, please make note on the drop off paperwork.

Photo and Video Release: By using our boarding services at VWCG, you agree to allow VWCG to use your pets name and any image taken during their stay with us. These may be used in any medica, marketing, advertising, or promotional material.

Fees: You must pay for all services and products by credit card, cash, or check. Unless a day board package is arranged,all payment is due at the time you pick up your pet unless the pick-up date is scheduled on a Sunday which must be prepaid. You also agree to pay any collections costs and the cost of any returned or challenged check or debit charges.

Abandoned pet: You may not leave your pet at VWCG after the scheduled pickup time without prior communication with VWCG team member. If you leave your pet without any contact, instruction, or notification regarding pick up, your pet will be deemed “Abandoned” at 7 P.M. on the seventh (7th) day after the scheduled pickup date. If your pet is Abandoned at our facility, VWCG will become its legal owner and guardian. VWCG will, in its sole discretion, determine whether to re-home the dog or relinquish it to a shelter or rescue of VWCG’s choice. You fully understand and agree that if you abandon your dog at VWCG, you may be unable to retrieve possession of your dog and will have no recourse against our facility.

Disclosure of Conditions: You must disclose on a continuing basis all medical or other conditions, including but not limited to personality concerns or behavioral. In the event of a material change to the information you provided in boarding check in sheet, you agree to submit any changes to the boarding team.

Veterinary Care: VWCG will have the right to obtain medical treatment for your pet, if, in VWCG’s sole discretion, it appears that your pet may be ill, injured or exhibits any other behavior that would reasonably suggest that your pet may need medical treatment. Reasonable efforts will be made to contact you when VWCG is concerned, but if VWCG is unable to contact you or your emergency contact, you hereby grant our veterinarian’s the right to make medically necessary decisions for your pet’s treatment and release the veterinarian and VWCG from all liability for the same. If you are unable to be reached but your emergency contact is reached, you hereby grant your emergency contact the right to make medically necessary decisions for your pet’s treatment. You agree to be fully responsible for all resulting costs related to any medical treatment.

By signing below, you represent and warrant that all the information provided to VWCG boarding is complete and accurate to the best of your knowledge. You further acknowledge and agree that you have read, understand, and agree to all the terms and conditions contained in the release above, as they may be amended from time to time. You further attest that if you are not the owner or sole owner of the pet(s) for whom this boarding entry sheet has been completed, your signature is sufficient to enter into this Agreement for and on behalf of any other owner or representative.
