I have enrolled in an online program offered by WomanUp Fitness. I recognize that it may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular and conditioning training as well as other various activities.
I hereby affirm that I have checked with my doctor prior to attending this specific live class. I am in good physical condition and do not have any known disabilities or conditions that would prevent me from participating in this event.
I hereby acknowledge that I am not pregnant at this time.
In consideration of this program, I hereby release WomanUp Fitness, it’s employees, and representatives from any liability now or in the future for conditions that I may obtain while on the premisis or after. I also release I hereby release all owners of the following property:
