TRAINING AGREEMENT AND LIABILITY RELEASE: I ("The Client") acknowledge and understand that Sage Canine, LLC reserves the right to deny or refuse a dog because of safety concerns with other dogs or employees. By using the services of Sage Canine, LLC and by submitting this form, The Client freely assumes all such risks, known and unknown, and the consequences thereof. The Client acknowledges and understands that there are certain risks involved in owning, training, caring for, handling, and being in the presence of dogs, including but not limited to injuries to people, property, and animals resulting from dog fights, dog bites to humans or other dogs and the transmission of disease and that these risks may increase when dogs are in unfamiliar surroundings or are in the presence of other dogs and strangers.

Dogs may be denied services due to physical illness. Dogs will be denied for human-directed, dog-directed, or animal-directed aggression unless given prior approval by Sage Canine, LLC. The Client agrees to maintain full control over their dog when present for services and accepts full responsibility for their dog's behavior. The Client affirms that their dog is current on all age-appropriate vaccinations. This includes but is not limited to Rabies, Distemper-Parvo or DHPP, Bordatella, and flea and parasite control. The Client's dogs must be free from infections, parasites, and contagious or transmissible diseases.

The Client recognizes the inherent risk associated with dog training and realizes that a dog is a creature capable of and given to independent action. The Client assumes the entire risk and responsibility for property damage and personal injury, harm, or death that may occur to their dog, themselves, their family, or any guests present as a result of the actions or inactions of the client, another person, or certain canine behaviors including, but not limited to, biting, scratching, knocking over, jumping on, or spreading transmittable diseases whether caused by client's dog or other dogs. The Client agrees to fully release and forever discharges Sage Canine, LLC, its agents and employees, from any and all claims for loss, damage, or injury to you or your dog(s) while in the care of or on the premises of Sage Canine, LLC or any of its partners. Care such as Puppy Grow Camp, Boarding, and Board & Train services which take place in the instructors home or while visiting other locations during field trips, training sessions, during transportation in a vehicle, private in-home or on-location dog training, workshops, group classes, events, outings, or any other services or activities offered by Sage Canine, LLC. The Client agrees to indemnify and hold harmless Sage Canine, LLC and its owners, employees, and agents from and against any and all claims, lawsuits, proceedings, losses, actions, liabilities, costs, and expenses, including reasonable attorneys' fees, incurred by such persons, which result from, arise out of or are in connection with the actions or inactions of The Client and their dog.

At Sage Canine, LLC's sole election, Sage Canine, LLC's duties hereunder shall terminate if (a) Sage Canine, LLC's sole judgment deems the client and/or dog is dangerous or vicious to Sage Canine, LLC or any other person or animal, or interferes with the training of other dogs, or (b) client breaches any term or condition of this agreement, or (c) in the case of safety concerns, financial concerns, unsanitary conditions, inhumane treatment of animals, non-payment of services, or situations that Sage Canine, LLC deems inappropriate or uncomfortable. Upon termination and in accordance with the foregoing Sage Canine, LLC's duties shall terminate, but all other provisions of the agreement shall continue in full effect.

The Client acknowledges and understands that there are no guarantees for behavior or skills and that refunds will not be issued for any services provided by Sage Canine, LLC, regardless of the outcome.

The Client shall be liable for all medical expenses and damages resulting from an injury to Sage Canine, LLC, its demonstration/staff dogs, and its staff caused by The Client's dog, as well as any damage to Sage Canine, LLC's property caused by The Client's dog. The Client agrees to indemnify, hold harmless, and defend the caretaker or instructor in the event of a claim by any person or animal injured by their dog.

CANCELLATION POLICY: The Client understands that service fees are due at the time of booking or before the requested service and are non-refundable. The Client must request to reschedule an appointment or service in writing at least 48 hours before the scheduled service. Rescheduling services must be agreed upon by both parties.

PHOTO AND VIDEO RELEASE: The Client consents to the use of photographs and videos of client/participant and/or their dog by Sage Canine, LLC, including, but not limited to publishing on Sage Canine, LLC's website and blog, all social media platforms including a Facebook page, Instagram account, YouTube channel or in email marketing.
