MEDALLION SELECTION
Choose from the pdf below the medallion(s) desired at the top of the plaque. Selected the matching code in the drop down menu. One medallion with a wreath is included in the base price of the plaque. The larger medallions are die cast metal and add an additional $25 to the order.
Center Medallion Selection:
It requesting a custom medallion, select "custom" and enter details in the notes section of this form.
Upper Left Header Position
Enter the description(s) of the graphics to the left of the medallion.
Upper Right Header Position
Enter the description of the graphics to the right of the center medallion.
Badges (indicate if engraved or mounted):
Descriptions of additional graphics/items (e.g. challenge coins, photos, graphics, etc.). Upload actual image files in upload section at end the form.
