Summer Camp 2021 Registration

1. I grant Gymboree Play Programs Inc. (GPPI) permission for my child(ren) to be photographed at the Gymboree Play & Music, Richmond, BC location during business hours and activities. 
2. I grant Gymboree Play Programs Inc. (GPPI) the right and permission to use the photographs and/or videos taken of my child(ren).
3. I shall have no right of approval, no claim to compensation and release GPPI from any claims arising out of the use of the photographs and/or videos taken.

By providing my electronic signature below, I understand and agree that this electronic form will remain in effect during the term of my child(ren)’s enrollment to Gymboree Play & Music, Richmond, BC.

I understand that in some situations medical attention may be required before Gymboree Play & Music, Richmond, BC is able to reach me. I voluntarily release any and all causes of action and/or claims against Gymboree Play & Music, Richmond, BC for personal injury, property damage and/or wrongful death resulting from such care.
