    Thank you for taking the time to complete this pre-appointment form prior to your pet's behaviour consultation. It is long, so please allow time (& save often!).
  • First, the practicalities:

    Your booking will be confirmed on receipt of your full fee, and all requested information - including [this] detailed Registration–History form; videos (including a narrated home tour, in the case of cats [and/or dogs - on request, as is relevant to the case]); +/- Veterinary Referral (for all behaviour cases) - details for all of which you will find in your Booking Confirmation Email.     This form is an important piece of the pre-appointment process. I know you may have filled in a long form previously, but it's been a while... Provision of lots of detail helps me to best help you & your pet.
  • Your pet's species *

  • Is your pet neutered (castrated or spayed)?*
  • At that time, was it for training issues (manners, life skills, fun stuff, socialisation, troubleshooting, etc.), or behaviour management (problem behaviours, referred by your vet, difficulty coping with your pet or their behaviour)?*
  • At that time we last worked together, did your veterinary practice refer you to Inspiring Pets? (I.e. did you send them a signed form to send to me?)*
  • Are there additional pets in your home?*
  • Are any other pets in involved in your behaviour/training concern?*
  • What is your above-named pet’s relationship status with any other family pets?*

  • Are there children in the home, &/or do children visit?*
  • Did you discuss training/behaviour with your vet? *
  • Does your pet have any current, ongoing, or past health conditions?*
  • Is your pet currently on veterinary-prescribed medication?*
  • Is your pet currently on non-prescribed, over-the-counter, complimentary or supplemental products?*
  • If your pet has health insurance, does this policy include cover for behavioural treatment?*
  • Is your insured to cover for 3rd party/public liability? (NB: this is highly recommended for ALL dogs)*
  • What does your pet normally eat from:*

  • Are you CURRENTLY seeking help for a perceived training or behavioural problem (please see our FAQs for further details on how to know this)?*
  • Do you give permission for Inspiring Pet Teaching’s trainer to feed your pet/give treats?*
  • Is your pet currently fed raw meat or foodstuffs, either in part/occasionally or entirely/frequently)*
