The applicant agrees to permit SBTC to obtain documentation of educational records, background check, and drug testing as needed. Additionally, the undersigned applicant permits their case manager/program supervisor/referring agency representative to share such documentation (if it exists) and any other information pertinent to applicant’s participation and goals.

El solicitante acepta permitir que SBTC obtenga documentación de registros educativos, verificación de antecedentes y pruebas de drogas según sea necesario. Además, el solicitante abajo firmante permite que su administrador de caso/supervisor de programa/representante de la agencia remitente comparta dicha documentación (si existe) y cualquier otra información pertinente a la participación y los objetivos del solicitante.
