BY SUBMITTING THIS FORM, YOU ARE HIRING BUBBULOSO FOAM PARTIES / THE CREATIVE PLACE LLC. TO BE AT YOUR PARTY ON THE DATE AND TIME PROVIDED. THIS FORM IS AN AGREEMENT AND NOT A QUOTE REQUEST.

• We require power and water hook-up within 75' from where you want the Foam.

If the distance is further than 75 feet you must provide additional hoses and extension cords. There will be NO REFUNDS whatsoever if we are unable to perform due to power or water non working at your location.*
