I grant Chadwell Supply, Inc. (Company) permission to use my story, name, likeness and any photographs of me, whether still or moving, in any and all of its publications, including online content and publication, without payment or other consideration. I understand and agree that I have submitted materials to Company voluntarily and they will become the property of Company and will not be returned to me.

I irrevocably authorize Company to edit, alter, copy, exhibit, publish, distribute or use my story, name, likeness or photos to promote the business of Company or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy in which my story, name, likeness or photos may appear.

Additionally, I understand that I will not receive any royalties or other compensation arising from or related to the use of my story, name, likeness and photos by Company. I hold harmless Company from any claims, demands, and causes of action which I, my heirs, representatives or any other person acting on my behalf or on behalf of my estate have or may have by reason of this authorization or the use of my story, name, likeness and photos. I certify by my signature that I am at least 18 years of age and am competent to contract in my own name.

Yo comprendo y estoy de acuerdo de que yo he sometido material a la Compañia voluntariamente y se convertira en la propiedad de la Compañia y no sera devuelto a mi persona.

Yo irrevocablemente autorizo a la Compañia a que edite, modifique, copie, exiba, publique, distribuya o use mi historia, nombre, semejanza o fotos para promover el negocio de la Compañia o cualquier otro proposito legitimo. En adicion, yo renuncio al derecho de inspeccionar o aprobar el producto finalizado, incluyendo una copia por escrito o electronica en el cual mi historia, nombre, semejanza o donde fotos puedan aparecer.

Adicionalmente, yo comprendo de que yo no voy a recibir ningun derecho u otras compensacionesque surjan o que esten relacionadas en el uso de mi historia, nombre, semanjanzas y fotografias en la Compañia.

Company grants Chadwell Supply, Inc. (“Chadwell Supply”) permission to use Company’s name, images, likenesses, logos, trademark, artwork, signage and photos whether still or moving, in any and all of its publications, including online content and publication, without payment or other consideration. Company understands and agrees that it has submitted materials to Chadwell Supply voluntarily and they will become the property of Chadwell Supply and will not be returned. Company irrevocably authorizes Chadwell Supply to edit, alter, copy, exhibit, publish, distribute or not use its Company name, likeness or photos to promote the business of Chadwell Supply. In addition, Company waives the right to inspect or approve the finished product, including written or electronic copy in which Company’s name, images, likeness, logos, trademark, artwork, signage or photos may appear.

Additionally, Company understands that it will not receive any royalties or other compensation arising from or related to the use of Company’s name, images, likenesses, logos, trademark, artwork, signage and photos by Chadwell Supply. Company holds harmless Chadwell Supply from any claims, demands, and causes of action which Company, its representatives or any other person acting on its behalf have or may have by reason of this authorization or the use of Company’s name, images, likenesses, logos, trademark, artwork, signage and photos.

The undersigned hereby warrants and represents that he/she has the full right and authority to enter into this agreement concerning Company’s name, images, likenesses, logos, signage and photos and that Chadwell Supply does not need the consent or permission of any other person, firm or corporation in order to use the Company name, images, likenesses, logos, signage or photos described above.
