  • NOTE: This Preliminary Registration form is automatically emailed to the registrar of the school you selected to inform them of your interest in enrolling your student. Once the initial information from this form has been entered into our system you will receive a phone call from the registrar. During that conversation, they will provide you with additional information about the school, answer questions, and provide instruction on how to complete the additional required forms using our Tyler SIS system.  Please call the school if you have not heard from them within 3 days of submitting this form.

    Students are not officially enrolled until such time as you have engaged in the phone conversation with our office and submitted all required forms.
