  • 1.) Authorization for Walking Excursions

    I give my permission for my child to go on occasional walking excursions around and away from the premises of DCP, under the supervision of DCP staff members.

  • 2.) Authorization for DCP to Use Photographs, Film, and Quotes

    I give my permission for DCP to photograph, film, and or record my child’s quotes for classroom use. I give permission for DCP to use photographs, films, photos of my child's art, or quotes from my child for various internal DCP publications, in publicity materials for the school, and/or on the DCP website and social media. I understand that my child's name will never be identified in any publicity materials, on the website, or on social media, but that it may be identified in certain internal DCP publications.

  • 3.) Authorization for Media to Photograph, Film, and Interview

    Permission for close-up photographs and interviews of children under 18 years of age can be granted to the media only with parental approval. These photographs, films, written or verbal interview quotes will appear in media articles or segments about DCP. I understand that DCP shall not be liable for any royalties, commissions, or payments of any nature to me or my children in connection with such filming, photographing, and/or interviewing. DCP assumes no liability of any nature in connection with such filming, photographing, and/or interviewing. I give my permission for DCP to allow my child to be filmed, photographed and interviewed by the media.

  • 4.) Authorization for Third-Party Assessment

    During the school year, DCP may wish to retain third-parties to perform observations and non-invasive assessments in order to gather information about a child's health and development, abilities, and behaviors to assist in better planning and support for meeting each child's individual needs. These third parties will use any information gathered during these assessments solely to assist DCP, the child, and the family. For example, DCP may retain third parties to assess a child's speech development and/or to observe a child's abilities and behaviors in the classroom. DCP or any of its staff will not arrange these observations or assessments without first discussing their concerns with the parent/guardian of the child in question. I give my permission for DCP to retain third-parties to assess my child during the school day, for DCP to disclose any relevant information to such third-parties, and for such third parties to provide, whether verbally or in writing, the results of their assessments to DCP (including but not limited to the Executive Director, the Assistant Director, my child's teachers) and to me. DCP assumes no liability and is hereby released from liability of any nature in connection with such assessments.

  • 5.) Authorization for Outdoor Play Without Shoes

    In the warmer months, children will often request to remove their shoes during outdoor play time, particularly when playing with sand, water, and mud. We are aware that our school is in an urban environment, and the educators and administrative staff regularly monitor the playgrounds for items that may be hazardous to children (i.e. glass, broken plastic, etc.) We do monitor this hazard to allow for the risk for children who desire to remove their shoes to be able to do so. Many children also benefit from the sensory input of experiencing the outdoors without shoes. We do understand that this decision is a personal one, and your preference may be that your child keeps their shoes on at all times. Please note, due to fire safety shoes must be worn during inside play at all times.
