    *This form will be considered annual consent for your pet(s)
  • Please THOROUGHLY read through our boarding policies and reservation information prior to clicking submit. 

  • Reservation Information

  • Reservations are necessary and space is limited. We recommend booking as far in advance as possible. Calls will be taken from 7:00 am - 6:00 pm M-F and Saturdays 8:00 am - 12:00 pm. If calls are received after hours, please leave a detailed voicemail and we will give you a prompt call the next business day.

     

  • Proof of Current Vaccinations

    These vaccinations are mandatory and for the benefit of your pet as well as any other boarders.
  • If your pet's vaccination records are being transfered from another hospital, it is highly recommended that you verify your records have been received prior to dropping your pet off for boarding. If our hospital does not have records of current vaccinations at the time of drop-off, the patient will be vaccinated for required vaccinations at the owner's expense.

  • Dogs:

    •  All dogs require proof of current Rabies, Distemper/Parvo, Bi-Valent Canine Influenza,Leptospirosis, and *Bordetella (within the last 6 months). 
      • *While we do understand that some veterinarians only require the bordetella vaccine every 12 months, in an effort to minimize the risk of kennel cough occuring and/or spreading, we require the vaccine to be boostered every 6 months for patients staying in our facility.
    • A negative fecal exam within the last 6 months. If your pet does not have one, our hospital will administer a fecal exam at the owner's expense.

    Cats:

    • All cats require proof of current Rabies and FVRCP vaccinations.
    • A negative fecal exam within the last 6 months. If your pet does not have one, our hospital will administer a fecal exam at the owner's expense.
  • What to Bring

  • Please bring your pet's food in a sealed plastic bag(s) portioned per individual meal labeled with your contact information and any medications currently being taken by your pet in its original container.

    If you would prefer your animal to eat our facility's food, please inform our EUVH team when booking your pet.

    Diets Fed:

    • Canine - Purina Pro Plan Prescription EN *dry kibble*
    • Feline - Hill's Science Diet Sensitive Skin & Stomach *dry kibble*

    If your animal is not wanting to eat while in our care, we will contact you to ask permission to add our hospital's *wet diets to your animal's food to entice them to eat. (*This will be charged per can to your account. Each can will be dedicated only to your pet and will not be shared amongst other boarding animals).

     You are welcome to bring special treats. We do not accept any other personal belongings with exception to medically necessary items (with additional signed waiver). Items we do not accept including but not limited to: beds, blankets, and toys. We provide bedding to all of our borders unless told by the owner they cannot receive them.

    We offer Kuranda cot beds, plush bedding and blankets.

  • Arrival

  • Upon arrival, your pet will be checked for fleas and intestinal parasites (if no fecal examination has been performed within the last 6 months with negative results). If fleas are found, your pet will be given a mandatory Capstar at the owner's expense. If you do not have record of a negative fecal exam within the last 6 months, one will be done at the time of drop-off at the owner's expense.

    If the results of a fecal exam are positive for intestinal parasites, an appropriate dewormer will be given to the pet at the owner's expense. We highly recommend and encourage the use of monthly flea/tick preventative as well as heartworm and intestinal parasite prevention. 

    Bathing is also available at the owner's request and expense. If your dog has any pre-existing conditions or behaviors we should know about, please take this opportunity to inform the staff upon check-in. 

  • Our Care Routine

  • Canines

    Our canine boarders are walked a minimum of 3 times daily with individual play time with at least 15-30 minutes per day.

    Food and medications are administered in the morning and evenings. Please let our team know if your animal has a special feeding or medication schedule.

    All kennels/runs are cleaned multiple times per day to ensure your pet is in a safe and clean environment.

    Water bowls are left available to your dog 24/7.

    Felines

    Feline boarders in our care get their litter and water changed twice daily to ensure they are kept in a clean environment. 

    Our kennel staff will check in with them throughout the day to give them individual attention. 

    Food and medications are administered in the morning and evenings. Please let our team know if your animal has a special feeding or medication schedule.

    Waiver and Release General Terms
  • Waiver and Release General Terms:

    A. Owner shall pay all costs and charges of and for special services listed in this agreement, and all veterinary and medical costs that may be incurred during the term of this agreement for the animal pursuant to this agreement.
    B. Owner certifies to the correctness of the information given above with respect to the animal and specifically agrees to be bound by the terms and conditions of this agreement.
    C. Owner specifically represents that (he/she) is the owner of the animal and that there is not now any lien or other encumbrance against the animal and that the animal has not been exposed to Rabies prior to admittance to the custody of East University Veterinary Hospital (EUVH).
    D. Social Media: We want to share your pet’s adorable pictures and story with others. Do you grant EUVH permission to post your pet’s picture and story on social media and our website?** Your personal information will not be shared. (Preference selection in following section)

     

    Liability:


    A. East University Veterinary Hospital (EUVH) and their respective owners, employees and agents, shall not be liable for any damages to the animal arising out of or from the boarding of the animal, or that may occur from any cause in connection with such boarding including loss by fire, theft, running away, death, or injury during the term of this agreement, whether the animal is on the premises of EUVH or not, except where the loss is caused by the gross negligence of EUVH, or its owners, employees or agents.
    B: Owner shall be solely responsible for all of the acts of the animal at any time during the term of this agreement, and in no case shall EUVH be liable for the animal's acts and behavior other than for the exercise of gross negligence on the part of the EUVH, its owners, employees or agents in the boarding and keeping of the animal. Owner shall indemnify EUVH from and against any and all damages sustained or suffered by reason of the boarding of the animal for any claims or injuries arising out of the boarding of the animal except where the injury is caused by the gross negligence of EUVH, or its owners, employees or agents.
    C. Owner agrees that EUVH will not be held responsible for aggravation of any pre-existing medical conditions. These include, but are not limited to, heart disease, arthritis, obesity, infections and any other conditions that might be discovered by examination.
    D. Owner agrees that EUVH does not assume and shall not be responsible for the loss and or destruction of personal belongings left with the pet while boarding, including, but not limited to, bedding, blankets, toys, dishes, collars or leashes.
    E. An animal in a group situation, such as boarding, grooming, dog park, training classes, etc. are exposed to common illnesses, similar to that of humans. Owner agrees to assume the risks and hazards that may be expected to arise from interaction with other animals or being exposed to such settings.


    Illness of Animal:


    A. In the event that the animal becomes ill, Owner shall be notified at once at the address or phone number listed above. However, if EUVH is unable to reach owner, if owner does not immediately inform EUVH regarding measures to be taken, or if the animal's health requires emergency action, owner grants EUVH the right to inform a veterinarian or to furnish other advisable attention within EUVH discretion, any expenses incurred in conjunction with such care is owner's responsibility and shall be promptly paid by owner.
    B. Owner agrees that (his/her) pet may sleep more than usual, drink more than usual and/or eat more than usual due to the change in his/her daily routine. Owner agrees not to hold EUVH liable for any illness or ailment that may affect (his/her) pet if (his/her)pet becomes ill during or after its visit and owner agrees to indemnify and hold EUVH harmless for any costs, damages, claims or expenses arising therefrom.
