Meetings of our mindfulness group are meant to provide individuals with information, training and personal support in order to help you be more fully in the moment, tolerate stress and regulate emotions.
Allyson Cole, PsyD and Sarah Freeze, LCSW, the facilitators, will at times provide general information about psychological and emotional issues, but will not engage in rendering psychological or healthcare advice for any individual or for his or her particular situation.
Because you may find it helpful, you may, but will not be required to, give personal information about yourself to other group members during the feedback portion of our meetings. However, I cannot assure you that other group members will keep private what is said in these group meetings. You assume that risk and understand that I cannot be held responsible for other group members revealing information about you that you may consider private or even sensitive. There are rules, however, that are meant to protect privacy. These rules, which you agree to follow, are: (i) you will take any measures necessary so that only your first name will be displayed during group meetings, (ii) you will not discuss any information about a group member except with that member and other group members during group meetings, and (iv) there will be no visitors at, or recordings of, group meetings allowed.
The mindfulness group does not provide medical, mental health or any other type of healthcare service. No diagnosis or treatment of, or advice regarding, any medical or mental health condition or illness of any group member will be offered during the training group. The mindfulness group cannot substitute for, and is not an alternative to, medical or other healthcare diagnosis and treatment when a medical or mental health condition or illness is present.
Because the mindfulness group is not a healthcare service, it is not reimbursable by any health insurance companies or plans.
Mindfulness group members are advised to seek diagnosis, treatment and advice regarding medical or mental health conditions or illnesses from physicians, psychologists and other licensed healthcare professionals. Mindfulness group members under the care of healthcare professionals for medical or mental health conditions are strongly advised to consult with such healthcare professionals about participating in the mindfulness group prior to joining the group.
Please sign and date below indicating you: (i) understand and agree to the purposes and limitations of the mindfulness group and (ii) are 18 years of age or older.
