Transcript Request Form

** This form is for STUDENTS ONLY to request copies of their transcript. If you are a 3rd party and are needing copies of transcripts, you must have the student request a copy to be sent to you or you can contact the school directly to do an education verification by emailing nikkimedley@joplinschools.org or at 417-680-0004. This form is considered fraudulent if it is filled out and signed by anyone other than the student named below.

Did you complete your program through Franklin Tech-Adult Education as an adult (18 years of age and up) or through Franklin Tech through the Joplin High School as a high school aged student? If you completed your program through the Joplin High School Franklin Tech, you will need to contact Joplin Schools at 417-625-5200 (ask for archives) to get a copy of your transcript since we are separate departments and we will not have a copy of your transcript.

If you completed your program through Franklin Tech-Adult Education as an adult student, please fill out the following form and we will get your request completed as soon as possible. Transcript requests take about 2 to 3 business days to be processed.
