Transcript Request Form

This form is for students only to request copies of their transcripts.

If you are a third party requesting a transcript, you must have the student submit a transcript request and have it sent directly to you. Alternatively, you may request an education verification by contacting the school at nikkimedley@joplinschools.org or by calling 417-680-0004.

Please note: This form is considered fraudulent if it is completed or signed by anyone other than the student whose transcript is being requested.

Did you complete your program through Franklin Technology Center–Adult Education as an adult student (18 years of age or older), or did you attend Franklin Technology Center through Joplin High School as a high school student?

If you completed your program through Franklin Technology Center at Joplin High School, please contact the high school from which you graduated. They can assist you with obtaining a copy of your high school transcript.
If you completed your program through Franklin Technology Center–Adult Education as an adult student, please complete the form below. We will process your request as quickly as possible. Transcript requests are typically processed within 3–5 business days.

Transcript Fees

A $5.00 processing fee is required for each transcript request.

Please submit one mailing address per request.
If you need transcripts mailed to multiple addresses, you must submit a separate request for each address along with the required $5.00 fee for each request.
Transcript fees must be paid in full before your request will be processed.
Requests received without payment will not be processed.

WE CANNOT SEND THEM ELECTRONICALLY.***Transcript requests that are received without paid fee will not be processed.***

Please upload a copy of the front of your photo ID for verification of identity so your record may be released. The photo must show the entire front side of the photo ID or it will be invalid and your transcript will not be released.
