Transcript Request Form

** This form is for STUDENTS ONLY to request copies of their transcript. If you are a 3rd party and are needing copies of transcripts, you must have the student request a copy to be sent to you or you can contact the school directly to do an education verification by emailing nikkimedley@joplinschools.org or at 417-680-0004. This form is considered fraudulent if it is filled out and signed by anyone other than the student named below.

Did you complete your program through Franklin Technology Center-Adult Education as an adult (18 years of age and up) or through Franklin Tech through the Joplin High School as a high school aged student? If you completed your program through the Joplin High School Franklin Tech, you will need to contact the high school you graduated from. They will be able to help you get a copy of your high school transcript.

If you completed your program through Franklin Technology Center-Adult Education as an adult student, please fill out the following form and we will get your request completed as soon as possible. Transcript requests take about 3 to 5 business days to be processed.

***There is a processing fee of $5 per copy/request. Please only list 1 address per request. If you need multiple transcripts mailed to multiple locations you must fill out a request for each address that it is to be mailed to along with a paid $5 fee for each request. Transcript fees must be paid in full before transcript request will be processed. Transcript requests that are received without paid fee will not be processed.***

Please upload a copy of the front of your photo ID for verification of identity so your record may be released. The photo must show the entire front side of the photo ID or it will be invalid and your transcript will not be released. *
