In case of an emergency we will contact parent/legal guardian 1 first, if we cannot get hold of this first contact we will contact the other parent/legal guardian. When both parents/legal guardians cannot be reached, we will contact the emergency contact provided to us in this form.
There is a yearly non-refundable registration fee of $100 (per family). This fee is waived when paying the tuition in full before the beginning of the school year and for expanding/changing family enrollment throughout the year.
For books and school supplies we charge a 'school materials fee' of $50 per student/per school year.
The regular tuition for the 2021 - 2022 school year will be $ 1935 per student and can be billed in 9 monthly terms starting September and ending in May. If you don't get a reimbursement from your employer for the tuition, we have grants available of $585 per student. This will bring your tuition for the school year down to $1350 per student. To apply for a grant, all you have to do is check the box 'my employer will not reimburse me' (the next question in this form).
DISCOUNT POLICY- Enrollment fee is waived for families that pay the tuition in full before the start of the school year.
Parents can use our Paypal facility to set-up a monthly autopay for Credit Cards and Paypal accounts. This incurrs a 2.2% fee on the amounts paid.
Most parents choose ACH Autopay using our ACH facility. Using ACH incurrs no additional costs and can also be used for contributions that vary by month, like private lessons. ACH is a bank system to automatically debit your bank account. We use Bill.com to process ACH payments.

CANCELATION FEE WHEN CANCELLING BEFORE THE END OF THE SCHOOL YEAR
If you cancel before the end of the school year, a cancellation period of 30 days applies, counted from the 1st day of a full month. School fees will be charged for these 30 days, even when students are no longer able to attend class. For example: when you e-mail on feb. 20 about cancelling for the school year per april 1st. The whole month of March will count as your cancelation fee and you will be out of school and costs per April 1st. However, if you cancel on feb. 20 with the notice that your child will stop classes per March 1st, you will still pay the whole month of March as your cancellation fee, even if your child does not attend class anymore.
CANCELLING BEFORE THE END OF THE SCHOOL YEAR, THAT YOU PAID IN ADVANCE
When you paid the tuition in full, before the start of the year, we will refund you the unused tuition months, minus the cancellation fee mentioned above. You will receive the refund within 30 days of your cancellation, counted from the 1st day of a full month.
The undersigned (the “Signer”), being the parent(s) and/or legal guardian(s) of the minor child(ren) (the “student”) named at the beginning of this form , for valuable and sufficient consideration, receipt of which is hereby acknowledged, on behalf of themselves and the Child do hereby knowingly and intentionally fully and irrevocably release, to the full extent permitted by California law, the Dutch School Silicon Valley (the “School”), and its board members, directors, officers, sponsors, administrators, staff (a.o. teachers and teaching assistants), employees, volunteers and agents, from any and all claims or liability, known or unknown, of any kind whatsoever, which may now exist, or at any time in the future arise, with regard to any matters relating, directly or indirectly, to the student's enrollment or attendance at the School, or to the Signer’s or the student's participation in any activities directly or indirectly related to, or sponsored or sanctioned by, the School, regardless of whether or not said activities occur on School premises or elsewhere. By our initials below and by our signatures at the end of this form, we knowingly waive and release any and all legal claims that we or the child(ren) may now or in the future have against the School, or anyone connected with it.
