I understand that this consultation intake form is for Jolene to determine if she’s the right stylist for my needs and does not guarantee an appointment. I acknowledge that the current wait time for new guest services is approximately 3–6 weeks. I agree that my first appointment requires an initial investment of $300, secured by a $100 deposit. Maintenance appointments range from $120–$175.
I understand that the salon has a strict 48 hour cancellation policy and if I do cancel my appointment with less than 48 hours notice my deposit will not be refunded.
