FALL PROGRAM OPT-OUT FORM

Junior, Cadette, Senior, and Ambassador troops may elect to opt-out of receiving recognitions in exchange for an additional 5% of each dollar sold. Girl Scouts in troops opting-out of recognitions ARE eligible to earn the Personalized Patch & Crossover Patch (if earned at the end of cookie season).
Please fill out the info below AFTER you have a UNANIMOUS decision from ALL of the girls in the troop. SUBMIT NO LATER THAN OCTOBER 20, 2023
