Design Consultation and Workshop Questionnaire
Thank you for your commitment to connecting children and families with the natural world. You’re on your way to creating a nurturing space for children! Your answers to the following questions will aid our collaborative design process and aid us in getting to know you, your participants, and your site in order to provide the best workshop experience.

Please allow approximately 20 minutes to complete the questionnaire. Provide information to the best of your ability. If there are design questions you cannot answer at this time, we will go over them together during our initial design meeting. Once you begin, you will not be able to save your work and return to it at a later time. It will need to be completed all at once. After it is submitted, you will receive a confirmation email that will include a link to be able to upload additional photos if more are requested at a later date.
