Filling out this form helps us prepare for your visit, ensuring we have the right products on hand and schedule adequate time for your service. Detailed answers are crucial for achieving your desired look and maintaining healthy hair.

Follow-Up Process: After receiving your form, we may send a follow-up email if further information is needed. If your appointment request is not accepted and you have not heard from us a week before the scheduled date, please contact us to confirm that your submission was received.

Reminder: Please ensure this form is submitted at least one week before your desired appointment date. If not filled out by then, your appointment request will be denied, and the slot will be opened to other clients.

Cancellation Policy: Some appointments require a deposit to be paid before scheduling. If you fail to show up for your appointment or cancel within 24 hours of your scheduled time, your deposit will be forfeited as a cancellation fee. To schedule another appointment, a new deposit will be required. If any appointments are pending without a new deposit, they will be cancelled. Should you decide not to receive services on the day of your appointment, your deposit will cover the cancellation fees, and any difference must be paid at checkout.*
