PLEASE READ THE FOLLOWING TERMS AND CONDITIONS BEFORE COMPLETING THE FORM:
This form is to ensure that my booking process is as fast & easy as possible for all parties involved. Please make sure to fully complete this form before submitting.

The more specific and direct your answers are, the better!

In addition to this form, you may be asked to attend a free face-to-face consultation prior to your tattoo appointment date.

I do NOT tattoo minors.

I also cannot ethically tattoo anyone who is currently pregnant or breastfeeding.

I will NEVER tattoo any overly offensve/racist text or symbols under any circumstances.

I charge an hourly rate of $120/hr for tattoo time.

All draw & prep time is included with the required $100 deposit. All deposits are non-refundable (but do of course come off of the total cost of the tattoo. Deposits can be sent through Venmo or dropped off in cash in order to secure an appointment date.) Please note that even if an appointment date has been discussed, your booking is not secure without a deposit.

All of my inks and supplies are vegan-friendly.
