XTD TRAVEL: Client Flight Submission Information Form
XTD Travel likes to maintain a full travel profile on all the clients/members who are travel with us on one of our vacation trips/travel events. Thus, if you procure your flight yourself but purchase your land package through XTD Travel, you must complete this form.

As this information is used to make sure your airport transfers (if included in your land package) can be arranged.

This form must be completed and submitted 45 days in advance of your scheduled trip departure date.

Important Form Completion Notes:

1. This form can be used for up to (4 Guests if you are all staying in the same room and flying on the same flight. If both of these statements are NOT true, you MUST complete separate flight information forms.

2. If you have additional guests (2 thru 4), you MUST provide a phone number and an email for airline contact tracing purposes. If the guest is a minor, please provide your email and phone number.
