ZUMBA IN THE PARK - LIABILITY WAIVER

· As an adult participant in any Health & Wellness, Community Service, or other events offered by Kabayan Inc.

I understand that it is my full responsibility to consult with a physician prior to and regarding participating in any Health & Wellness, Community Service, or other events or Programs. I represent and warrant that I am physically fit and have no medical condition that would prevent my full participation in the event.

o In consideration of being permitted to participate in any events offered by Kabayan Inc., I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in the program.

o In consideration of being permitted to participate in any events offered by Kabayan Inc., I knowingly, voluntarily, and expressly waive any claim(s) that I, my heirs or legal representatives may have against Kabayan Inc. of Matsu, their Boards of Directors, Board Members, General Members, insurers, or any event coordinator(s) or Instructor(s) for injury or damages that I may sustain as a result of participating in the program.

I, my heirs or legal representatives forever release, waive, discharge, and covenant not to sue Kabayan Inc. of Matsu, their Boards of Directors, Board Members, General Members, insurers, or any event coordinator(s) or Instructor(s) any injury or death caused by negligence or other acts.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
