I am the assigned Team Coach, and have the agreement of the four players (and their parents) as listed above.

On behalf of the team, I will provide an LTA Accredited + Coach to be on site for the duration of the event.

I agree to adhere to the social distancing and covid related measures, as detailed in the 'Official guidelines" which will be issued to players, coaches and guests 5 days prior to the first day of the event.

I understand that, as a condition of entry, all coaches, players, staff and guests visiting the site will be required to complete and return a self-declaration (health and travel) and upload a passport style photo in advance of the tournament
