I understand and accept that there are risks to participating in activities of the Edmonton Table Tennis Club (“ETTC”). These risks include, but are not limited to, the risk of physical injuries, pain, suffering, temporary or permanent disability, economic loss, death or the risk of contracting communicable diseases. After full consideration, I agree to accept any and all such risks.  

I accept that I am fully responsible for my own safety and actions. In case of injury or illness, I give my consent to emergency transportation and administration of first aid, medical and/or dental treatment.

I accept responsibility for the payment of any emergency transportation, treatment expenses and any related or subsequent medical and/or dental bills. I acknowledge that ETTC has not purchased and does not provide any medical or accident insurance to cover such expenses. Any such insurance is my responsibility.

I agree that any photographs, audio or video recordings of me by representatives of ETTC are the property of ETTC and may be used at their discretion.

I agree to be subject to the current ETTC Bylaws.

I understand that I may be disqualified from further participation, without notice, if I am deemed by the Board of Directors to be contravening the bylaws.

I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in ETTC and agree to hold harmless the ETTC, its members, officers, directors, employees, volunteers, agents or any other representatives, from any and all causes of action, claims, demands, losses or expenses, however caused, that result from my participation in or travel to or from the ETTC or any ETTC managed or sponsored activity.
