I understand that this is a general consultation intake form designed so Danielle can help me choose which color session best suits my hair goal.

I understand that if I desire a more thorough explanation of the coloring process, or a detailed analysis of my current hair I will need to schedule an in-person consultation.

I understand that by submitting this form it does not guarantee that my goal hair can be achieved in one session.

I understand that by submitting this form it does not mean I have an appointment booked. I understand that the average wait time to get on the schedule for a session is around 4 weeks.

I understand that the initial investment for my first appointment ranges from $270-$450 and maintenance/session appointments range from $175-$300 depending on current hair, goal hair, and required upkeep.
