OCCUPANT'S NOTICE OF INTENT TO TERMINATE RENTAL AGREEMENT

On the date this Agreement is dated, the "Ocupant" hereby gives Menlo-Atherton Storage (MAS) Notice of Termination of:

Please Note: Vacate form expires 90 days after the "Expected Date of Termination" if the Occupant does not vacate the space listed above.

• Occupant shall have removed all belongings from the MAS facility by the end of the day of the "Date of Termination". If they do not move out, after 90 days the vacate form will expire and Occupant will have to submit new form.
• OCCUPANT SHALL NOT LEAVE TRASH, DEBRIS, OR OTHER DISCARDED MATERIALS in or about the rental space or the MAS facility. The rental space shall be left UNLOCKED and completely empty. Further, tenant shall notify MAS of any damage left in space.
• Your Rental Agreement REQUIRES A MINIMUM OF THIRTY (30) DAYS PRIOR WRITTEN NOTICE TO TERMINATE TENANCY. The Thirty (30) day period shall not start until written notification is received by MAS. Notices sent after MAS business hours(10: AM to 5:00 PM Tuesday through Saturday excluding Sundays, Mondays & Holidays) shall be credited as received on the next MAS business day. The contract continues to be in full force until the unit is vacated and ALL RENTS CONTINUE TO BE DUE during the notice period.
• MAS shall refund rent and/or other fees due to Occupant within thirty (30) days after Occupant relinquishes rental space as per the provisions of the MAS Rental Agreement. Occupant's mailing address to send refund to:
