    Puppy Questionnaire

**All answers will remain confidential and for adoption purposes only**

All puppies produced by Elhart K-9's are temperament tested and hand picked to match with their family.  We will only place puppies with families we feel will be a match and compliment your experience, lifestyle, animals in the home.

In filling out the puppy questionnaire with as much accuracy as possible, this helps us get to know you better in order for a match to be a complete sucess!

Please bear in mind that our dogs are not all the same and while one of the Elhart dogs may catch your eye; their drive or temperament may or may/not be suitable for your family.  i.e. high drive/high prey drive sire and dam may produce these traits and wouldn't be suitable for famiiles with small children/cats/horses/cattle etc. but excel as working/sport/show dogs.  Elhart has purposely created their breeding program to have different levels of dogs in energy and drive/intelligence/trainability so we can choose a litter that is best suited for applicants.  We also have quieter/lower drive dogs here that are more suitable for the first time dog owner or families that do not have as much experience with dogs.

In filling out this application and submitting, you are agreeing that you will be willing to spend the time and money needed to feed, house, train and provide medical and emotional care to your dog.

    • Completing this application is a step in the process to acquiring a puppy from Elhart K-9's German Shepherd Dogs, but it does not guarantee I'll get a puppy from this or any future litter. While it's unlikely, the breeder reserves the right to refuse any placement at any time.

APPROVAL AND DEPOSIT: I will review applications within 2 business weeks of receipt of application. Applications are processed in the order they are received. We breed based on our need to add another dog to our kennel or on demand from our clients, so we will only accept deposits when puppies are available. Once the litter has been born, approved homes will receive a DEPOSIT FORM to review/sign and a deposit of $250.00 is required to hold a puppy.  Please make this deposit within 48 hours of the approval e-mail. If you make a deposit, you are committing to take a puppy from the litter information provided.

PICK OF PUPPY: Puppies will be temperament tested at day 49 and then families will be matched by Elhart after this date. If Elhart is holding back puppies for their breeding program and wanting to evaluate puppies for a longer period of time, there may be a delay up to 10 to 12 weeks of age before puppies are placed with their families.  During this time, all puppies will continue crate, manners, house breaking and socilization training at no further cost to the buyer. If the puppies are evaluated and there is not a puppy that will be a good fit for your family, there are options to either:

      a) wait on a future litter

      b) request the deposit back

If there is a puppy available to you but you decide not to proceed, there is no refund on the deposit.

GOING HOME: We cannot hold puppies past the take home date provided, unless agreed upon by both parities in writing with additional costs noted for boarding and training.  This option for each litter is based on first come, first serve basis.

If you have a big vacation planned or have events that would conflict with a young puppy, please know puppies cannot be boarded in a kennel until they have received their full immunizations and their rabies vaccination which cannot be done until 3 months of age.

CONTRACT: Elhart K-9's requires all puppies to be placed under a signed contract which we call a Health Guarantee.  We believe in providing as much information as possible and being upfront with our potential families in order to insure our puppies are placed in safe and loving homes. If you have questions about specific terms, please contact us prior to putting a deposit on a puppy.

SPAY/NEUTER: We require spay/neuter at or before 18 months.  We have a take-back provision in our contract. Any puppy born from our kennel always has a home here, and will never experience the fear and burden of life at a shelter. Taking back an unaltered dog into our home poses significant risk to our own dogs due to canine STDs and the possiblity an unaltered dog could be pregnant. Elhart may require testing/screening of the dog prior to accepting the dog back at our kennel. This would be determined on a case by case basis.  Please inquire if you need further information.

MICROCHIP/VET/REGISTRATION PAPERS: All of our puppies are microchipped, vet checked by a licensed veternarian and provided wormings and first shots prior to leaving our kennel.

All Elhart litters are registered with the Canadian Kennel Club (CKC).  Litters and puppies will be registered however the delays of these registrations are at the mercy of CKC dependant on if the litter was imported or whelped at Elhart.
