WWTA WAIVER/RELEASE FORM

By participating, volunteering or spectating at Western Wake Tennis Association (WWTA) programs and events, I acknowledge the risks associated with playing tennis, accept those risks voluntarily and agree to hold harmless the Western Wake Tennis Association (WWTA), the host and host facility, the instructor and their employees and/or agents against any claims of any kind of injury or loss suffered at the WWTA program or event. I understand that no insurance coverage is provided by WWTA or host facilities. I waive and release any and all rights and claims against the WWTA and their partners and any other participating municipalities and/or their employees for any injury or loss suffered while taking part in any WWTA program and/or event.

I also understand that despite care taken, a decision to participate in the events may increase my risk of contracting Coronavirus (COVID-19) among other contagious/infectious syndromes/or diseases. Participant, volunteer, or spectator knows and assumes all responsibility for all such risks.

In addition, I agree to allow any photographs/videos to be taken of me to be used for promotional purposes by WWTA, Abilities Tennis Association of North Carolina, Wheel Serve of North Carolina, other WWTA partners or host facilities for their social media. Social media includes but is not limited to newsletters, website, Facebook, Twitter or Instagram.
