EMERGENCY CONSENT FORM
It is the policy of this Centre to notify a parent when a child is ill or needs medical attention. Occasionally we cannot contact parents and we need to get immediate help for the child. Our procedure is to take the child to the nearest emergency service.

Please sign below so that we can take appropriate action on behalf of your child. Return the signed card to the Centre immediately.
I hereby give consent for my child/ren
when ill to be taken to the nearest emergency centre by the staff of Beth Tikvah Congregation and Centre Association when I cannot be contacted. I consent to an ambulance being called to transport the child, if necessary.

I understand that I assume all risks and hazards incidental to the conduct of Beth Tikvah activities, and hereby release, absolve, indemnify and hold harmless Beth Tikvah Congregation and Centre Association, directors, supervisors and employees from all claims or injuries arising there from.

Photo/Video Consent and Release Form
I hereby grant permission to Beth Tikvah and its volunteers to photograph, take video or otherwise record images of my child(ren), for use by BT. I further grant to BT and its volunteers the right to reproduce, use, exhibit, display, broadcast, publish and distribute these images and recordings in any media now known or later developed (including but not limited to the internet and the website at btikvah.ca, which may be accessible anywhere) for promotional, publicity, fundraising, and teaching purposes, including explaining BT and its activities, without consideration or compensation to me or my child(ren).

I certify that our participation is voluntary and that I enter into this Consent and Release on my own behalf and on behalf of my minor child(ren), as applicable.

Grade 3 – Grade 7*	$970	$1,607 or $1,570 (cash)
One time B’nai Mitzvah fee** For the entire preparation, classes addition, and a personal Siddur B'Nai Mitzvah students who do not attend a Jewish Day School will attend on Saturday, Sunday and Tuesday.	$1450	$3,275 0r $3,200 (cash)
For twins, the entire package will be	$3,070 or $3,000 (cash)
Annual registration/supply fee	$20	$20

One month’s written notice is required before withdrawal from the program. In lieu of notice, the following month’s payment will be kept.

*Tuesday classes will run from 3:45 pm - 5:45 pm.

**The one time B'nai Mitzvah fee can be paid in grade 6 or 7.
