    2. LEASE ADDENDUM: An addendum to modify the lease will be drafted and sent out for signautres.

    3. APPLICABLE FEES & PET RENT: Per the lease agreement, fees will be charged and and/or adjusted to your tenant portal.

    NOTE: This form is to remove an animal only. If you are requsting to replace by adding a new animal, please also complete the "Request for Adding an Animal Form"
