Research participants information, data management and informed consent for Stakeholder Forum members
Dear participant,

in order to join the activities planned for TRUSTEE Stakeholders Forum (SF), please take a moment to read the Ethics and GDPR Information Sheet provided for all Stakeholder Form participants, as required for data protection and participation purposes. After that, please fill in all the sections of the form and sign to give your consent.

It will take less than 5 minutes, we thank you in advance for helping us in complying with GDPR and ethics duties before we start stakeholders involvement activities!

INFORMED CONSENT FORM
I declare that I have read and understood the Participant Information Sheet, including the Ethics and GDPR information, for my role in the Stakeholder Forum.
I understand that my participation in the Stakeholder Forum is voluntary and that I may withdraw at any time without any negative consequences, penalty or prejudice, and without needing to provide a reason.
I understand how my contributions during Stakeholder Forum activities (surveys, interviews, workshops) will be used for research purposes and that the results will be reported in anonymised and aggregated way (please take care to not provde sensitive information).
I acknowledge that no potential risks and burdens associated with my participation have been identified, and that appropriate measures have been taken to minimise them in the case any risk or budens might arise.
I agree to respect the confidentiality of information shared within the Stakeholder Forum and not to disclose sensitive information about other participants or organisations outside the agreed framework.
I am aware that the information, knowledge and opinion I will provide will remain confidential within the group of participants and no sensitive information will be distributed outside the project.
I understand that I will not receive any compensation or incentive for having taken part in planned activities.
I confirm that I have had the opportunity to ask questions about the research, the ethics implications, and the processing of my data, and that all my questions have been answered to my satisfaction.
