Waiver and Release

Sports House LLC Consent, Waiver, and Release

I hereby consent to my participation in the Sports House Training Center (“SHTC”). I represent that First Name* Last Name* is qualified, in good health, and in proper physical condition to participate in such activity.

I hereby release, forever discharge, covenant not to sue, and agree to save and hold harmless SHTC, William, Daphne, and Cody Jones from all liability, claims, demands, losses, or damages on my account, caused by, or alleged to be caused by, in whole or in part, the action, inaction and/or negligence of SHTC, William, Daphne, and Cody Jones and further agree, that if I or anyone acting on my behalf makes a claim against SHTC, William, Daphne, and Cody Jones that I will indemnify, save and hold harmless SHTC, William, Daphne, and Cody Jones from any litigation expense, attorney’s fees, loss, liability, damages, or cost incurred as a result of such claim.

In the event that First Name* Last Name* is injured while participating in SHTC, and I am not available to take responsibility for treatment, I authorize SHTC, William, Daphne, and Cody Jones to consent to any medical or dental treatment recommended by an appropriate medical or dental professional and I agree to pay for any cost or expenses of treatment rendered pursuant to this authorization.

In the event that I cause damages or break a piece of equipment in or on the property of SHTC or on the property owned by William Jones, I agree that I am responsible for the cost of a full replacement.
By signing below you are also accepting our cancellation policy. This policy states that we can only offer a 50% refund if athlete cancels within 48 hours. We do not offer a 100% refund unless the event is completely canceled.
