Adoption fees are listed at the end of this application.

If you choose to give up, rehome, or abandon your animal adopted from Passionate Paws Dog Rescue at any time for any reason, we require that they come back to us. All dogs adopted out from Passionate Paws Dog Rescue have been seen and cleared by a licensed vet. If your animal becomes sick or injured after adoption, you may contact us, but the cost of care after adoption is the sole responsibility of the party who adopted the animal. Passionate Paws thanks you for choosing adoption and saving a life.

All adopting parties are required to complete this form prior to adoption. I also understand that this adoption process may take several days. Passionate Paws Board members are all strictly volunteers.
