You can enter up to five subcontractors using this form. You'll need to have this information ready for all your subs:

You can only enter five subcontractors with this form. Submit additional subs with the List: Subcontractors form. You can use the same form to list subcontractors later in the construction process.
Choosing the Submit button on the next screen constitutes your digital signature on the application. By submitting this application, you are acknowledging an obligation to provide information needed to determine compliance with local construction and zoning codes as well as an obligation to ensure that work is performed by appropriately licensed persons or persons exempt from licensing.
What happens next?
Your application will be routed to our Permit Center and you will receive an email confirming your application. Most of the transaction will be handled by email unless a phone call is necessary. Payment of fees can be handled in a number of ways.
When we receive your application, our first step will be to determine if you are eligible to apply for the permit. Then we will review what you submitted to determine if the application is complete and we will request additional information if necessary (we usually need to).
