    Please fill out one contract for each pet that will be staying with us. Note that this is a boarding contract only and does not secure your pet's boarding reservation.     I hereby entrust The Playground to care for my pet while boarding and agree to all of the terms listed below. If I am unavailable to pick up my pet(s) past the date reserved, I understand my pet(s) may be moved to an alternate space if my originally booked space is no longer available. I authorize Lafayette Veterinary Care Center to do what they deem necessary for the health and well-being of my pet(s) and agree to pay for any and all expenses relating to said services. I am aware that my pet, despite being vaccinated, could still be susceptible to contracting airborne illnesses such as kennel cough. To maintain a flea-free environment for all guests, every pet is examined at check-in. If fleas are detected, treatment will be administered at the owner’s expense. I agree to pay the daily boarding rate of $42/day for pet(s) staying in a loft/condo and daycare charges of $26/day.

    *Please note that all medications must be in their original packaging. Oral or topical medications will be administered free of charge. We are not able to administer insulin injections. If your pet does require insulin please contact the office to arrange medical boarding at our hospital.

    *Please note that Lafayette Veterinary Care Center provides food and food bowls for every pet. We serve Purina EN which is a prescription diet for sensitive tummies.

  • I agree that I am solely responsible for any damages that the above mentioned pet(s) may cause through malicious or improper conduct. I also agree to give permission for my pet(s) to have unrestricted and unsupervised access to my personal belongings listed on the front page during their stay at Lafayette Veterinary Care Center. Therefore, I agree to release Lafayette Veterinary Care Center from any liability of damages or harm that my pet may cause to my personal belongings or himself during their stay. In the event where my pet is dismissed from doggie daycare, LVCC reserves the right to transfer my pet to the resort for the remainder of their stay. I have read this agreement on this date, listed below, understood its terms and signed it freely.
