Parent Consent - Curriculum Content: Participants will engage in activities and conversations that will help them navigate topics that youth navigate such as suicide prevention, substance prevention, and abstinence. Topics may be complicated and delicates but are supported by evidence-based practices that help to build coping and resiliency skills.
Confidentiality: Student confidentiality is protected by the Family Educational Rights and Privacy Act ("FERPA"). What a mentor hears and observes about a student, family and staff while mentoring in and outside a school is confidential. Repeating a seemingly harmless comment can lead to misunderstandings and hurt feelings. For schools to provide the best environment for learning, everyone’s privacy must be respected.
Child Neglect and Abuse Reporting: Mentors are encouraged to report any suspected child neglect or abuse. If a mentor suspects child abuse, he or she must immediately contact the program director.
Termination of Agreement: Parent(s)/guardian(s) can stop the mentoring of their child at anytime by verbal and or written request to the mentor or any school official. Parent(s)/guardian(s) are also to notify 3D Girls Inc of this termination in writing or via email at firstname.lastname@example.org.
