We/l, the owner or authorized agent of the property listed above, hereby authorize AFC Cleaning & Restoration LLC/American Floor Care LLC, herein known as "Contractor" to enter the property and perform emergency/mitigation services. We/l agree to pay any cost incurred not covered by my/our insurance. We/l also authorize my/our insurance company to direct pay to contractor for the emergency/mitigation services and repairs performed, or include contractor's name on the face of the check or draft for the mitigation services and repairs performed. By signing this emergency/mitigation work authorization, we/l meet the requirements in the insurance policy listed above to protect the property from further damages, and make the necessary and reasonable repairs to protect it. The contractor agrees to keep record of services performed and provide insurance company a copy of the reports and invoice.
We/l agree to hold harmless the contractor for draperies, clothing, carpeting, upholstered goods, etc. that cannot be cleaned or may become damaged. We/l also agree to hold harmless the contractor for any illness resulting from prior exposure to mold or poor air quality which existed before the contractor began services. Contractor STRONGLY ADVISES clients to vacate the property while mitigation work is being performed and until property is cleared as having acceptable air quality. Contractor further advises anyone who has any health-related symptoms to contact their healthcare provider immediately for diagnosis and treatment.
