CALIFORNIA USBC YOUTH HONOR AWARD FORM
Achievements must be bowled in a California USBC certified youth league or youth tournament. Recipient must live and be members of a California youth USBC certified league. A COPY OF THE RECAP AND FRAME BY FRAME PRINT OUTS (scratch) MUST BE SUBMITTED. Only games 1-2-3 of a set are valid for series awards or 4-5-6 if it's 6-game block. Applications must be submitted within 30 days of the achievement. Allow 3-4 weeks for processing. Email us at info@calusbc.com with any questions. NOTE: ONLY ONE AWARD PER BOWLER, LIFETIME PROGRESSIVE as submitted.
