GUIDELINES
- After submission of this online form, please allow us to confirm your appointment request within 3 to 8 hours. We will be sending an E-Certificate confirming your appointment and payment. This will be sent to your inbox within 3 to 8 hours.
- Please do not forget to bring the following: E-Certificate, 1 copy of accomplished CIF forms and a photocopy of your valid government issued ID as well as your Senior Citizen and PWD ID, if any. The CIF form will be sent to your e-mail after submission of this appointment request.
- For cash and credit card payments, please wait for confirmation of your appointment request. You may settle your payment at the swab testing facility before or on the day of your scheduled appointment.
- On the day of your appointment, please be 15 minutes early. Show your E-certificate to our guard for your queue card, then our parking attendant will be providing the necessary forms.
- Our trained technologists will perform the swab test. Your official results will be sent via e-mail.
