VOLUNTEERING: If you are a returning 7th - 12th grade student, would you be interested in assisting with younger classes? You would receive 40 volunteer hours at the end of the season, wonderful for National Honors Society, college, & scholarship applications.*

If a student does not acclimate into a particular class, or if a class has low enrollment and cannot run, PA5678 reserves the right to recommend a different class. If the alternate class(es) do not fit your schedule, you will receive a full refund.*

Prepaid Yearly Tuition - 1 time payment in full, WAIVED $40 Administration FeeSemi-Yearly Tuition - 2 payments, twice a year; 50% due upon enrollment and 50% due on January2, 2026; Administration Fee DISCOUNTED to $30.September/October Deposit & Tuition Installment Plan (6 monthly payments made November-April) - spreads out the cost of yearly tuition. Payment is due on the 1st of each month. September/October payments and costume deposits are due upon enrollment. The remainder of yearly tuition is divided up, with monthly payments November - April. Please note: Payment is not for the number of classes each month. You are paying equal increments towards the total tuition due for the full season. (If payments are late, after two late payments, the remaining balance will be due on the 1st of the next month.) SHOES & ATTIRE: It is important everyone dress appropriately. DANCE: Students should wear appropriate dance clothes and dance shoes. MUSICAL THEATRE: Dance clothes or leggings/tank-tops, jazz shoes. ACTING: Any attire that's easy to move in.*

I understand. ATTIRE: I understand the importance of "dressing the part," and acknowledge that everyone has improved focus, posture, and overall demeanor and work ethic when dressed appropriately. ANY color/style of dance clothes is okay. SHOES: I understand that dance shoes should NOT be worn outdoors, and students should arrive in "street" shoes and change into their dance shoes upon arrival. On our website, under "Class Descriptions", specific shoes are listed. (Just as students would never show up to swimming lessons without a bathing suit, please be sure students have appropriate attire for their PA5678 classes.)

MEDICAL WAIVER & COVID INFORMATION:

COVID INFORMATION: Performing Arts 5678 follows the rules and guidelines set forth by the state of NJ. As NJ adjusts rules, regulations, and guidelines, Performing Arts 5678 will adjust accordingly.

MEDICAL WAIVER: I, the legal guardian of the student(s), have registered at Performing Arts 5678, agree to the following conditions: I recognize that participation in programs may occassionaly lead to injury. I authorize Performing Arts 5678 to contact the persons named as guardians and to authorize a physician to render treatment to my child as deemed necessary. I assume full financial responsibility for medical care for the participant while he/she is enrolled in Performing Arts 5678.

I, for myself and behalf of my student participant, also release waive and discharge Performing Arts 5678, all of its affiliates and related entities, and its officers, employees and representatives and their estates and/or heirs, from any and all claims which we may have for damages to property, personal injuries, medical situations or death as a result of participating in the programs sponsored by Performing Arts 5678. This releases and waives all claims which have happened and/or may happen as a result of participating in programming associated with Performing Arts 5678.

BODILY INJURY RELEASE Student and his/her/they heirs, distributes, guardians, legal representatives, and assigns HEREBY WAIVE ANY AND ALL RIGHTS, LIABILITY, DAMAGES AND/OR CLAIMS AGAINST THE RELEASED PARTIES FOR BODILY INJURY OR PROPERTY DAMAGE WHICH MAY OCCUR DURING CLASS, REHEARSAL, AND/OR PERFORMANCE, AND HEREBY RELEASE AND FOREVER DISCHARGE THE RELEASED PARTIES, AND EACH OF THE FROM ALL CLAIMS ACTIONS, DEMANDS, RIGHTS, CAUSES OF ACTION AND LIABILITIES, EITHER BY LAW OR IN EQUITY, based on bodily injury or property damage to Student or any third party as a consequence of Student's participation in class, rehearsals, or performances, or as a result of negligence (orginary or gross negligence) by PERFORMING ARTS 5678. Student hereby agrees that it will never bring any legal action against PERFORMING ARTS 5678.

REFUND POLICY Prorated Tuition refunds will be issued after the first class, in the event that a student cannot continue. Refunds must be requested in writing, via email, no later than September 25, 2024. Refunds will be made in the form of a check. If costumes have not been ordered, costume deposits will also be refunded. Once costumes are ordered, costume costs are non-refundable. If you would like a refund prior to classes beginning, you must request via email. Cancellation requests must be made no later than August 25, 2023.
