GROUP On-Demand Course Registration Form
- This registration form is for GROUP on-demand courses. If you instead want to register for an INDIVIDUAL on-demand course, click here.
- To register for a course(s): Select a course(s), complete required fields, and click "SUBMIT".
- Designate an employee(s) (the course proctor) to manage each group training session. Management of a group training session includes sign-in sheets, handouts, and surveys.
- Course proctor will be enrolled in and receive access to the course in 1-2 business days (usually sooner) after submission of this form. Course proctor will be notified of enrollment via email.
- Course proctor will have access to the course for at least 2 weeks after enrollment. Course access can be extended or renewed upon request.
- Course can be presented through any internet-connected device (computer, tablet, or smartphone).
- Course must be presented on a TV or monitor large enough and loud enough for your employees to see and hear it.
- At the conclusion of each training session, the course proctor MUST scan and email sign-in sheets and surveys to ATA. No course credit can be given without this step.
- You will receive course completion certificates within 2 weeks after submitting sign-in sheets and other course documents, and will be invoiced at the beginning of the following month.
