Thank you for taking the time to fill out our new client application form. We have been overwhelmed with requests to take on new clients. In an effort to ensure we can service our patients effectively we must review each potential new client before we take them on as a client.
Waiting Period
We will contact your previous veterinary clinic to request records to be sent. We will then contact you once we have space open up which may take some time. We understand that in the meantime you may need to keep searching for a new veterinary clinic.
Cancellation Policy
A $45 cancelation fee will be incurred if you are unable to provide 48 business hours notice prior to your appointment.
Payment
We will gladly prepare a written Treatment Plan prior to any appointment/procedure. We accept cash, debit, Visa & MasterCard. We do NOT accept personal cheques. ALL PAYMENT IS DUE WHEN SERVICES ARE RENDERED – PLEASE NOTE THAT WE DO NOT EXTEND CREDIT
Covid-19 Protocol
A friendly reminder we are still only offering curbside services at this time and are only accepting payment over the phone. If you have been experiencing any COVID-19 symptoms or have been exposed to anyone who may have been, please reschedule your appointment.
