As a district, we often have the opportunity to take photographs of children engaged in learning. We are requesting your permission to use such photographs of your child in our publications. Those publications could include but are not limited to newsletters, the district website, the yearbooks, and local press releases.
Illinois now requires a performance assessment to insure pre-service teachers (student teachers) have the necessary knowledge, skills and dispositions to be effective educators before teaching licenses are issued. As part of this Teacher Performance Assessment (edTPA), pre-service teachers must video their performances and the impact their instruction is having on student learning. Students and their voices must be recorded and student work samples scanned as evidence that learning is occurring. To protect their identity, students' last ofthe teaching names are not used on the recording or on any documents submitted to the team of evaluators performances. Parents, who do not want their children recorded or their children's work samples scanned, should contact the principal in writing annually.
Please check ONE of the following three options regarding consent to use your child's photographs and/or video. If you do not return this form completed and signed, your child's photograph will not be published in any publications, including the yearbook.
1. Full Permission to Use Photographs and Video: As the parent/guardian of the above named student, I give my permission for the Lebanon Community Unit School District #9 to use photographs and videography of my child to illustrate the educational activities of District #9 in communications such as, but not limited to newsletters, the district website, the yearbooks, local press releases and Teacher Performance Assessments.
2. Yearbook-Only Option: As the parent/guardian of the above named student, I give my permission for the Lebanon Community Unit School District #9 to use photographs of my child only in the yearbook. This permission does not extend to any other publications of the District or Teacher Performance Assessments
3. No Permission to Use Photographs and Video: As the parent/guardian of the above named student, I do not give my permission for the Lebanon Community Unit School District #9 to use photographs of my child in any publications including yearbook.
