This form must be completed and either scanned and uploaded using the following field, or a completed hard copy delivered to Discovery Child Care Centre prior to your childs start date.
If YES, please provide a copy of the appropriate legal documentation (i.e. court order)
BY SIGNING BELOW, I HEREBY CONSENT TO DISCOVERY:
(i) All of the personal information included in any Application/Enrollment/Registration Form that I may complete and give to the Facility about the Child(ren), me and any other individual whose personal information is included in the forms, and
(ii) Any additional personal information regarding the Child(ren), me and/or any such other individual that the Facility may reasonably require and collect by way of additional documents and/or interviews/meetings;
2. Collecting all of the personal information included in any Subsidy Application/Financial Assistance Form that I may complete and/or give to the Facility about the Child(ren), me and any other individual whose personal information is included in that Subsidy Form, and forwarding that Subsidy Form to the appropriate government office for review by them;
3. Collecting any additional personal information included in any other document that I may complete and/or give to the Facility about the Child(ren), me and any other individual whose personal information is included in that document - for example: a Child and/or Family Information Record; a Pre-Authorized Payments Form; a Custody Arrangements Agreement; a Medication Administration/Authorization Form; an Emergency Medical Attention Form; a Daily Care Record; a Food Allergy Action Plan, and any Special Needs Form, (collectively, the "Other Forms", and with the Registration Form, any Subsidy Form and all Other Forms being herein collectively called the "Forms");
4. Collecting, on an ongoing basis, additional personal information about the Child(ren), me and any other individual who is referred to in any Form (a "Named Individual"), as reasonably required at any time by the Facility to provide for:
(i) The physical, emotional, social and intellectual well-being and safety of the Child(ren), or
(ii) The general administration and operation of the Facility (which includes record keeping, debt collection and fund-raising activities);
5. Using all of the personal information included in any Form and all other personal information previously or subsequently collected by the Facility regarding the Child(ren), me and/or any other Named Individual, as reasonably required by the Facility to provide for:
(ii) The general administration and operation of the Facility (which includes record keeping, debt collection and fund-raising activities);
6. Disclosing, on a “need to know” and, where applicable, on a confidential basis, any personal information about the Child(ren), me and/or any Named Individual:
(i) To the Facility's employees, independent contractors (i.e. people who regularly work at the Facility but who are not paid as employees), directors, therapists, volunteers (which may include parents and/or legal guardians of other children attending the Facility), work experience students and educational or regulatory observers; or
(ii) To any third party who requires such information in order to:
a) provide (or assist the Facility in providing) for the physical, emotional, social and/or intellectual well-being and/or safety of the Child(ren); or
b) assist the Facility in its general administration and/or operations (which includes record keeping, debt collection and fund-raising activities); or
c) assist the Facility in providing products and/or services to the Child(ren) and/or to me; or
(iii) To a public authority or an agent of a public authority if, in the Facility’s reasonable judgment, it appears that there is imminent danger to life or property which could be avoided or minimized by the disclosure of that information; or
(iv) To any third party who requires such information and who is part of the Facility's organizational group; or
(v) To any third party with whom the Facility is negotiating for the purpose of that third party taking over some or all of the Facility's services and/or other activities; or
(vi) To representatives of other licensed child care facilities if I have not promptly satisfied my debts to the Facility;
7. Disclosing the first name of the Child(ren) in any class list produced by the Facility for the Child(ren)'s class(es) at the Facility and made available to families of children in each class/time slot;
8. Disclosing the Child(ren)’s first name and the first initial of the Child(ren)’s surname on a name tag (whenever other children in the Facility are given name tags) on the Child(ren)’s clothing and on various items throughout the Facility – for example, a sign-in/sign-out sheet, an allergy sheet, a cubical, a locker and a notebook/scrapbook;
9. Allowing photographs and/or videos of the Child(ren) to be taken by, staff of the facility, therapists, by work experience students and by educational or regulatory observers while they are at the Facility;
10. Taking photographs and/or videos (or allowing appropriate photographs and/or videos to be taken) of the Child(ren) and using any such photograph and/or video in the Facility's program activities, pedagogical documentation such as Journey Books, Floor Books & Documentation Panels, Google Classroom, archives and/or promotional materials (including but not limited to: newsletters, website, Facebook, Twitter, YouTube, and other social media; and
11. Allowing photographs and/or videos of the Child(ren) to be taken by:
(i) Employees of the Facility for classroom activities; or
(ii) Representatives of the media or of any granting organization, for use in any media coverage, article or promotional materials; or
(iii) Parents of other children at the Facility at special events or field trips organized by the Facility.
Also by signing below, I acknowledge that the Facility may collect and disclose personal information regarding the Child(ren), me and/or any Named Individual, without the consent of the Child(ren), me or that Named Individual, where:
a) the Facility is required by law to make any such disclosure (i.e to comply with licensing conditions); or
b) a staff member of the Facility suspects abuse, neglect or endangerment involving the Child(ren)
THE TERMS OF THIS AGREEMENT ARE AS FOLLOWS:
REGISTRATION FEE: The parent/guardian agrees to pay a registration fee of $50 for the 1st child and $25 for each additional child. This fee is payable at the time of registration and is non-refundable.
DEPOSIT: The parent/guardian agrees to pay Discovery a deposit of two (2) weeks fees at the time of registration. This deposit will be held without interest and will be applied to the last two weeks that the child is in the centre. The deposit will be updated annually to match current fees. The parent/guardian agrees to provide, in writing, two weeks’ notice of withdrawal (10 full business days) and understands that failure to comply will result in forfeiture of the deposit monies. 10 business days does NOT include the 2 weeks we are closed (Last week of July & the week between Christmas & New Year’s). The parent/guardian further agrees to give two weeks’ notice, in writing, to change the child’s program or hours of attendance. The deposit will not be refunded if the child fails to attend the program registered for if two weeks written (10 full business days) notice is not received.
SUBSIDY AGREEMENT: If subsidy for childcare fees is obtained from the County of Simcoe, the monthly fee paid by parents is based on the assessed daily fee contribution. Parents are responsible for fulfilling all the requirements of the County of Simcoe necessary to maintain the subsidy. If the parent becomes ineligible for childcare subsidy, they are responsible for paying the full monthly fee, including 2 weeks withdrawal notice (10 business days) based on non-subsidized child care rates.
FEES: Fees are due on the 1st of each month and is to be paid by email transfer to email@example.com. A late fee of 24%/annum, calculated monthly will be charged for outstanding balances (minimum $10/month). There are no deductions in fees for absences including: statutory holidays, illness, vacation or Discovery’s closings whether or not Discovery has given notice.
RECEIPTS: Discovery agrees to provide a receipt for the total amount paid in fees during the year. Receipts will be held in the office for pick up from February 1st – June 30th of the year the receipt is dated. A fee of $25 will be charged to process tax receipts lost or not claimed by the 30th of June of the year the receipt is dated.
ARRIVAL & DEPARTURE: The parent/guardian agrees to take the child directly to the staff member on duty when the child arrives. Children will not be accepted into care after 11am. The staff member must acknowledge receipt of your child before Discovery can accept responsibility for the child. With respect to departure, the parent/guardian agrees to inform the staff for the child’s class when he or she is picking up the child from Discovery. The parent/guardian agrees that after the parent/guardian has advised the staff that he or she is picking up the child, Discovery is no longer responsible for the child regardless of whether the child remains on the premises. Parents MUST supervise their children and must ensure that their child remains with them at all times.
ILLNESS: The parent/guardian agrees to follow and adhere to all of Discovery’s Illness Policies as outlined in the Family Handbook received at time of registration & DCCC Covid-19 Response Plan (as amended). The parent/guardian and Discovery agree that if in the opinion of the staff, the child has a temperature; flu like symptoms or a communicable disease, the child will not be accepted by Discovery. If the child becomes ill during the day, the parent agrees to pick up the child immediately upon receipt of notice of the child’s illness. The parent agrees to keep the child home for 1 full day, symptom free without medication after the sickness. The parent/guardian understands that their child will not be admitted into care if they have been given any fever reducing medication for any reason in the past 8 hours. This policy helps to control the spread of childhood illnesses to other children and staff. Should parents not pick up their child when requested to do so, or does not comply with our illness policies, they may be asked to leave Discovery permanently.
PERMISSION FOR EMERGENCY MEDICAL ATTENTION: If I cannot be reached in an event of an accident or other medical emergency, I give permission for the staff of Discovery to obtain immediate qualified medical assistance for my children named on the front page of this enrollment package. It is understood that every effort will be made to contact me immediately. I also accept responsibility for any expenses incurred for whatever immediate medical assistance is required as determined by the staff of Discovery.
I hereby release Discovery Child Care Centre Inc. and its employees of any liability with respect to actions they may take in executing the authority given by this form.
CLOSINGS: The parent/guardian understands that Discovery is closed for all statutory holidays, Easter Monday and the August Civic holiday, (As per Family Handbook). Discovery also reserves the right to close the centre when deemed necessary (i.e. power failures, snow days etc.) There will be no reduction of fees for these closures. Discovery also closes at 12 noon on Christmas Eve (Dec 24th). All Discovery-Centres will close the last week of July every year and will reopen the day after the August Civic Holiday as well as the week between Christmas Day and New Year’s Day every year. Monthly fees are based on 50 weeks/year so parents are not paying for the two weeks we are closed for holidays.
FIELD TRIPS: Field trips can be wonderful learning opportunities. We will take the children on field trips occasionally throughout the year. Prior to any trips off the child care property that requires transportation, parents will receive a field trip permission form. This form needs to be completed by the parents to give consent for the trip. Due to staff limitations, childcare will not be available to children not participating on the trip. There will be no reduction in fees for children not participating in trip.
PERMISSION FOR NEIGHBOURHOOD WALKS: I give permission for my children enrolled at Discovery, to go on any walking field trips supervised by Discovery staff. I understand that these field trips will be short walks around the neighbourhood and to local playgrounds/parks. I understand further that I will be notified in advance and in writing about any longer trips requiring transportation. Due to staff limitations, childcare will not be available to children not participating on the trip. There will be no reduction of fees for children not participating on any trips.
LATE CHARGES: Late fees of $15/child for the first 15 minutes and $1/child per minute thereafter will apply to children not picked up by 11:30 am for nursery school program or 6pm for all other programs. Late fees will also apply to children not picked up by 12pm on December 24th. Families will receive an invoice that will be due upon receipt and can be paid by email transfer.
TERMINATION OF SERVICES: There may be instances when Discovery cannot accommodate the ongoing or future needs of a currently enrolled or wait-listed child. In the event it is determined that the program at Discovery does not meet the needs of the child, Discovery reserves the right to require the withdrawal of the child from the program or waitlist.
Discovery may terminate child care services immediately in such circumstances as:
PERMISSION TO APPLY SUNSCREEN: I agree to apply sunscreen to my child prior to dropping my child off in the morning. I will be providing sunscreen for my child and understand that it must be peanut/nut free, non-aerosol, & labelled with my child's first/last name
I allow the staff of Discovery Child Care Centre to apply the sunscreen I supply on my child as per manufacturer’s instructions.
MEALS & SNACKS & ACCOMODATING ALLERGIES: Discovery will provide lunch & 2 snacks daily to children enrolled in our full day programs with the exception of the school-age program. A note from your family doctor or allergist may be required for us to accommodate allergies & special diets. Discovery must be informed in writing, of any changes to your child's allergies. While Discovery Child Care will attempt to minimize the risk of any known allergens, it is not possible to guarantee the environment is free of it/them.
NSF CHARGES: All payments which fail to clear the bank for any reason are subject to a $35 service fee. Replacement payments must be made by Interact within 5 business days. I understand that failure to do so may result in my child’s care being suspended or terminated and that there may be an additional set up costs of $50 to reinstate my child’s care. A late fee of 24%/annum (minimum $10/month), calculated monthly will be charged for outstanding balances.
The parent/guardian agrees to compensate Discovery for any damages caused by their child and indemnify Discovery for any claims arising as a result of their child’s actions or of their failure to abide by the Agreement.
The parent/guardian agree to supply as needed: Extra clothes, formula, baby food, and other specialty foods not provided by Discovery. There are additional fees for field trips, guest speakers/visitors.
This Agreement is subject to change by Discovery if deemed necessary and advisable to the best interest of Discovery upon two weeks’ notice. Written notification will be given by Discovery for increases in child care fees.
EMAIL CONSENT: I give permission for Discovery to distribute centre related newsletters, reminders, program information etc. to my email address provided below (or n/a if left blank).
I agree to the terms and conditions as outlined above. Further, I have received and read a copy of the Family Handbook (June 2021 version) and understand and agree to abide by all policies as contained therein and read. I understand that I am entitled to independent legal advice prior to signing this Agreement.
We all have the right to be safe and feel safe in our school community. The Discovery Code of Conduct sets clear standards of behaviour that apply to all individuals involved in our centre community including parents or guardians, volunteers, teachers, and other staff.
These standards apply whether they are on centre property or at centre-sponsored events and activities.
All members of the centre’s community are to be treated with respect and dignity regardless of race, creed, sexual orientation, disability or any other ground protected by Ontario’s Human Rights Code
All adult members have the responsibility to act as models of good behaviour. Foul language, swearing, name-calling, shouting is not appropriate. Individuals engaging in such behaviour will be asked to leave the premises immediately.
Inappropriate behaviour or harassment of any kind towards a student, parent or teacher will result in immediate intervention up to and including the family’s expulsion from the centre and/or police intervention. This type of behaviour includes but is not limited to harassment or intimidation by written note, email, social media, words, gestures and/or body language.
No weapons are allowed on centre property or at centre functions. The consequences for failure to comply will include but is not limited to the family’s expulsion from the centre.
Alcohol and illicit drugs are not allowed on centre property or at centre sponsored events. The consequences for failure to comply will include but is not limited to the family’s expulsion from the centre.
The privacy and confidentiality of our parents, guardians, teachers, volunteers and students is important to us. All concerns and comments should be addressed with the teachers. Should this discussion not address your concerns, the next step is to review the situation with the Supervisor and/or Director of Child Care.
Gossip and public criticism are unacceptable. There should be no discussion of concerns with other parents in the centre hallways, the parking lot or via electronic mediums such as Facebook, Instagram, personal blog sites or other forms of electronic information sharing or Social Media.
Any pictures taken at the centre or during centre events are for the private use of the family only. These pictures cannot be posted in on-line photo albums (i.e. Instagram Facebook, Twitter etc.)
School cubbies are to be used solely for the purpose of communicating between parents and Discovery. They are not to be used for business promotion.
Please refrain from asking staff to babysit or tutor your children. Please refrain from “friending” any staff on social media sites like Facebook, Instagram etc. as this would go against company policy and The College of Early Childhood Educators Code of Ethics/Standards of Practice. Staff are not permitted to accept friend requests from families or babysit for clients of Discovery.
This code of conduct must be signed by any and all adults that will be involved in your child’s experience at Discovery including parents, grandparents, siblings and care givers.
I have read the Code of Conduct and agree to terms as stated. I have been given the opportunity to review this document, ask questions if required, and confirm that no further clarification is necessary.
