During the 2021-22 school year your child may need to take medication while at school. The attached form must be signed by the physician prescribing the medication and must accompany the medication to school. This applies to non- prescription as well as prescription medication (Tylenol, Ibuprofen, cough drops, etc Prescription medications need to be in the container labeled by a pharmacist with the child's name, name of the medication, dosage, and directions on the label. Non-prescription medication must be in the original container with the ingredients listed and the child's name on the container.
Students with asthma must have a completed medication request form on file, as well as, a completed Asthma Action Plan. These students are able to carry their inhalers with them, as long as, the doctor and the parent have signed stating that the child has been instructed in the use and self-administration of the medication.
