The 2021-22 school year will be here before long. The State of Illinois requires all incoming pre- kindergarten, kindergarten, sixth and ninth grade students to have a physical examination prior to entrance into these grades. Please note the "Certificate of Child Health Examination" form must be used. This is a two sided form-
information must be completed on both sides of the form. Parents are required to complete and sign the health history section of the form. Your child's doctor must complete all required sections (see attached code Your child will not be able to attend school until all required fields are completed on the physical form. A one page physical will not be accepted. **The athletic physical is not sufficient for entrance into 6th or 9th grade. The required immunizations are:
DTP/DT - 4 or more doses, at least 4 weeks apart, with the last dose received on or after the 4th birthday. Students entering 6th-12th grade are required to have 1 dose of Tdap vaccine (combined tetanus, diphtheria, acellular pertussis) regardless of the interval since the last DTaP, DT, or Td dose. IPV - 4 or more doses with the last dose received on or after the 4th birthday. MMR (Measles, Mumps, Rubella) - students K-12th; 2 doses - the first dose received on or after the 1st birthday and the second dose no less than 4 weeks later. Students in Pre-K are required to have 1 dose received on or after the 1st birthday. Varicella (Chickenpox) - 2 doses of the vaccine with the second dose received no less than 4 weeks after the first dose. Hepatitis B - 3 doses, interval of at least 4 weeks between 1st and 2nd dose, interval of at least 2 months between 2rd and 3rd dose. The interval between the 1st and 3rd dose must be at least 4 months. Hib - (Pre-K) primary series according to Hib schedule or proof of receiving 1 dose at 15 months of age or older. Pneumococcal vaccine - (Pre-K) according to schedule. Meningococcalconjugate vaccine - Students entering 6th-11th grade: the dose must be given on or after the 11th birthday. Students entering 12th grade shall show proof of having received two doses of the vaccine prior to entering 12th grade. The first dose shall have been received on or after the 11th birthday and the second dose received on or after the 16th birthday, at least eight weeks after the first dose. If the first dose is administered at 16 years of age or older, only one dose is required.
Completed physical exams and documented immunizations are to be presented to the school office
PRIOR TO THE FIRST DAY OF SCHOOL. YOUR CHILD WILL NOT BE ALLOWED TO ATTEND SCHOOL UNTIL STATE REQUIRED PHYSICAL EXAMS AND IMMUNIZATIONS ARE COMPLETED.
A DENTAL EXAMINATION (state requirement) is required for children entering kindergarten, second, sixth, and ninth grade. To comply with the State Law, your child must have this exam between Nov. 15, 2020 and May 15, 2022. This completed form must be turned in to the office by May 15, 2022. Dental Safari Company, a mobile dental corporation that travels to schools to provide wellness checkups and urgent care treatment, is scheduled to come to our school during the 202-2022 school year. An EYE EXAMINATION (state requirement) is required for all children entering kindergarten or upon first entry into an Illinois school beyond kindergarten. This exam is due by Oct. 15 and must be performed by a licensed optometrist or a medical doctor who performs eye examinations.
