Commission Slots Notification

I made this form to simplify things, so all entries here will automatically generate a card on my Trello board. Notification emails and/or DMs will be sent at least a week or two before reopening or earlier if there are changes to my prices and/or terms. Please do check your spam/junk folders as well since my emails might end up there!
