SHOWCASE PERFORMANCE - If you are a registered dancer and have a choreographed dance piece that you would like to have considered for the Saturday evening showcase, please provide more detailed information. Please Note: There will be a limited number of dance pieces that can be considered for the showcase. Submit your application for consideration early. Once we receive your submission, we will send instructions on how to submit your video. An application will not be considered without providing a video of the choreography.

WAIVER The waiver must be submitted for each participant. Parents of students under the age of 18 should initial their acknowledgement and agreement.

I hereby release the OKC Tap Fest, Metropolitan School of Dance, Inc. (MSDI), the instructors, artists, sponsors, affiliates, promoters and facility management from any and all claims of liability due to personal injury or loss of property which (my child or I) may sustain as a result of participating in an activity connected with OKC Tap Fest. In addition, I acknowledge and understand that the teaching faculty and class schedule may be subject to change.

I state that I (and my child) will comply with the rules and regulations set up by the OKC Tap Fest and MSDI. I further acknowledge and understand that both OKC Tap Fest and MSDI have the right to refuse service to any person at its own discretion.

I agree to allow the OKC Tap Fest and MSDI to use any video or
photographic pictures taken, of myself (or of my child), for any promotional and/or archival purpose. Please leave blank if you do not want your or your child’s pictures shared publicly. We will honor this decision.

Teacher Discount - Teachers, we value you and what you do for your students. As our Thank You to you, for every student that registers and lists you as their teaching instructor, you will receive a $10 credit that goes towards your OKC Tap Fest registration.
