Happy Picasso Art Studio COVID-19 Liability Release Waiver

In consideration of my participation in the foregoing, the undersigned acknowledge and agree to the following:

I ( or my child )am aware of the existence of the risk on my physical appearance to the venue and my participation to the activity of the Organization that may cause injury or illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.I( or my child)have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.

By signing below I( or my child) acknowledge that I ( or my child) have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my ( or my child) consent; That I ( or my child)have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I ( or my child)give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.

I, the undersigned parent or legal guardian of [ First Name ] [ Last Name ] (hereinafter, my “Child”), voluntarily assume all such risks and agree that the Studio and its affiliates shall not be liable for any damages, injuries, or losses that may occur as a result of my participation.Parents or legal guardians of students shall not hold Happy Picasso or the rented facility owner liable for any accidents or injuries sustained before, during or after class or outside the property. Parents or legal guardians waive the right to any legal action to the foregoing. All risks will be assumed by the parents or legal guardians

Release of Liability

Although the safety and well being of your child is our top priority at Happy Picasso ,we, ( owner, volunteers, consultants, and staff) will not be responsible for and will be forever RELEASED from any injury(ies), permanent disability or death, claims, and/or any other liability arising from your child’s attendance and participation in our services/classes and programs at Happy Picasso, and/or for any lost property left behind by your child. By signing this waiver form, you agree to indemnify and hold harmless from any and all injury(ies),permanent disability or death, claims, causes of action, and/or any other liability arising from your child’s attendance and participation in our services/classes/camps and programs the above-mentioned RELEASES at Happy Picasso Art Studio .If any provision of this waiver and release is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.
